Live Well at Home

PACE KC offers you an alternative to placing your loved one in a nursing home, so they can remain close to friends and family. By providing all the help you need, including hands-on clinical and social support, we can help give you peace of mind that they are living a life they love.

We Provide It All

PACE KC offers complete management and coordination of your loved one’s care. This includes primary and specialty care, physical and occupational therapy, social worker support, in-home support for some daily living activities, and recreational activities and socialization in our Adult Wellness Center. For medical and PACE center appointments, we also provide transportation.

Lower Costs &  Stress

PACE KC simplifies the management of care and lowers costs. It is available at $0 plan cost to participants who qualify.*

Our program can reduce the financial stress of care for you and your loved one. We provide assistance with entitlements, so you can maximize your loved one’s benefits.

 

We Keep You Informed and Involved

Nobody can replace the care and connection of a loving family relationship. But by providing the healthcare and services your loved one needs, we can reduce your stress, lower your care burden, and help your relationship become even stronger. We don't replace the family connection, but we can enhance it. A team dedicated to your family member’s care stays in close communication with you and coordinates updates. Families participate in care plan discussions and in providing some care at home.

*All PACE KC services are included and paid for directly by Medicare and Medicaid benefits for qualified participants. There is no additional cost for any covered service, as long as it is provided by an in-network provider. Participants may be subject to a MOHealthNet (Medicaid) spend down as a condition of participation.
