Coordinated, personalized care

One of the things that makes the PACE model unique is our integrated Interdisciplinary Team. Ourteam of specialists meets every day to discuss our participants care needs and to develop individualized care plans to meet their health needs. Here are some of the members of PACE KC's coordinated care team.

David Hathaway - Registered Dietitian Nutritionist - PACE KC

David Hathaway, RD, LD
Registered Dietitian Nutritionist

I am a Registered and Licensed Dietitian with over five years of clinical experience in both hospital and outpatient settings. At PACE KC, I provide medical nutrition therapy to support chronic disease management, lead nutrition education for participants and their families, and work closely with the interdisciplinary team to ensure safe, nutritious, and enjoyable meals. I also work to address food insecurity and supporting the overall wellness of our participants. Outside of work, I enjoy traveling, cooking, bodybuilding, exploring new cuisines, and spending time with my family and myTibetan Terrier, Eli.

Physical Therapist

I bring  over 30 years of clinical and leadership experience in physical therapy to PACE KC. With a strong background in orthopedics, manual therapy, and geriatric rehabilitation, I am passionate about helping older adults maintain independence, mobility, and quality of life. Throughout my career, I have led multidisciplinary teams, developed clinical programs, and mentored providers—all with a focus on delivering exceptional, person-centered care. At PACE KC, I  advocate for functional wellness and equitable access to rehabilitative services in the community, consistently delivered with a smile and a love for my work.

Zenaida Klafta
Life Enrichment Specialist

I am a Certified Activities Director in the State of Missouri and a Certified Laughter Yoga Instructor, with 24 years of experience. As a Life Enrichment Specialist at PACE KC, I design and facilitate comprehensive activity programming that supports the cognitive, emotional, and social well-being of older adults. I integrate evidence-based practices and person-centered approaches into daily programming, tailoring activities to align with each participant’s care goals and cultural background. My work emphasizes dignity, connection, and quality of life, and I collaborate closely with interdisciplinary teams to ensure programming remains dynamic, inclusive, and clinically supportive.

Primary Care Physician

I am a specialist in geriatric medicine with over 40 years experience. I graduated from University of Missouri–Kansas City School of Medicine in 1985 and completed a residency at Deaconess Hospital St Louis University. I also speak Spanish and Tagalog.

Clinic Manager

I was born in Vietnam during the time of war and came to America in the 70s. My family were sponsored by a local Kansas City church which housed us and introduced my family to America. I started school here not knowing an ounce of English and was in a ESL class for majority of my primary school years. I graduated school with a degree in Respiratory Therapy which I practiced for about 10 years. I went back to school to become an nurse which I have been practicing for 17 years.

I have had previous experiences in other PACE programs as a Care Manager doing mostly patient care. I am currently the Clinic Manager at PACE KC. I really enjoy being part of the PACE programs as I see it helping the poorest, neediest, and underserved populations and making an impact on their lives.

Nurse Practitioner

I have 20+ years of experience as a dedicated healthcare professional in a variety of roles. This experience enhances my ability to anticipate of needs of aging adults. I attempt to make “care” visible by taking a passion, having pride, and prioritizing participant-centered care.  In my spare time, I enjoy fishing, gardening, and camping with my family.

Wathaiya Mungai - Social Worker - PACE KC

Wathaiya Mungai, MSW
Social Worker

It is my passion as a Social Worker and human to advance the rights of historically marginalized people, empower folks to practice autonomy and self-determination, and to provide care to others in a holistic manner. For the last 13 years, I achieved that through my work with children and families. I hope to utilize that same framework in my work with PACE KC participants. A few other areas I am passionate about are Neurodivergence and challenging societal norms, conspiracy theories, and personal development. When I am not focused at work, you can find me spending time with my partner and kids, cooking, and watching the latest reality TV show.

Day Center Manager

With over 15 years of experience in healthcare and management, I serve as the Day Center Manager at PACE KC. I’m passionate about supporting our community—especially the geriatric population, whose wisdom and warmth continually inspire me. Giving back and making a positive impact is at the heart of everything I do.
