We welcome referrals from healthcare providers and social service professionals. We make it easy to refer people to PACE KC with three convenient ways to refer.
Who is eligible for PACE KC?
To be eligible for PACE KC, participants must:
- Be 55 years old or older
- Live in Jackson County, MO
- Require nursing home level of care*
- Be able to live safely at home with support
* Participants must be certified by MO HealthNet as meeting the Level of Care requirements. Our Enrollment Specialists can help potential participants determine if they are likely to qualify.
