Our interdisciplinary care team members serve as your personal health navigators to deliver comprehensive and high-quality medical care, as well as much more.
As a valued PACE KC participant, you will have access to primary care, rehabilitation therapy, and other routine medical services at the PACE KC Adult Wellness Center. Additionally, you can participate in PACE KC classes, outings, fitness sessions, meals, and recreational activities that promote socialization and an active lifestyle. With PACE KC, you can enjoy a wide range of services that support your physical, emotional, and social well-being.

Coordinated care
At PACE KC, we believe in treating the whole person, addressing your physical, emotional, and psychological needs. Our interdisciplinary team of medical and senior care professionals meets daily to make sure that you can meet your health and wellness goals. Our customized, coordinated approach to care reduces the stress and hassle of having to coordinate services across medical providers and organizations, lifting a huge burden from you and any caregivers you have.
No additional costs
Once you enroll in PACE KC, we are resonsible for all of your care costs. You will never pay anything extra for any of the included services as along as:
- The service is included in your plan of care
- The service is received from an in network provider
- The service is medically necessary
- The service is approved by your PACE KC Team
